Non-Clearance Instructions Related to the College’s Health and Safety Protocols

Applicability: This Questionnaire is required by all staff, faculty, and vendors reporting to Claremont McKenna College.

Individual did not consent to the CMC Return Symptom Check Questionnaire.

  • You are not permitted on campus.
  • Please return home, and contact your healthcare provider to obtain documentation of your readiness to return on-site.
  • Please also check in with your direct supervisor to inform them that you do not yet have clearance to return to campus.
  • Continue to work remotely or report the absence according to College policy.
  • A member of Hamilton Health Box will follow-up with you to discuss next steps.

Individual was not given clearance to return to work based on information provided in the CMC Returns Health Screening process.

  • You are not permitted on campus
  • Please return home, and contact your healthcare provider to address your symptoms and/or obtain documentation of your readiness to return on-site. In the meantime, please follow the CDC self-isolation protocols in the information provided to you.
  • Please also check in with your direct supervisor to inform them that you are not permitted to return to campus at this time.
  • Continue to work remotely or report the absence according to College policy.
  • A member of Hamilton Health Box will follow-up with you to discuss next steps.

Individual developed symptoms while working on campus.

  • You will need to leave campus.
  • Please return home, and contact your healthcare provider to address your symptoms and/or obtain documentation of your readiness to return on-site. In the meantime, please follow the CDC self-isolation protocols in the information provided to you.
